This role focuses on empowering patients to manage their chronic diseases effectively while ensuring high-quality care delivery. The ideal candidate will conduct comprehensive assessments, assist in developing care plans, and provide patient-centered education and support. Candidates must be certified Medical Assistants (MA) with a strong background in chronic disease management.
